This role provides primary care inside a skilled nursing facility
under the ISNP program, managing a consistent panel and focusing on
keeping residents in place and out of the hospital. What’s in it
for you Monday–Friday, typically 4 days rounding 1 admin day
Flexible daytime schedule — structure your day as long as residents
are seen and charting is completed No nights, no weekends required
for daily rounding Call availability: Monday–Friday, 8 AM–5 PM only
(after-hours covered separately) Admin day is remote, includes
charting time and team meetings Mileage reimbursement full
benefits: medical, dental, vision, 401(k) match, HSA, PTO
Compensation Base Salary: ~$120,000 $4,000 annual bonus Per-visit
or additional bonus structures may be discussed as census grows —
higher earning potential with established patient panel. What
You’ll Do Provide longitudinal primary care to ~75–80 long-term
care residents per month (panel-based) Round onsite 4 days per week
between two facilities (typically Warren & Marienville) One remote
admin day weekly Manage both chronic and acute conditions Focus on
“treat-in-place” care models to prevent unnecessary
hospitalizations Coordinate with facility staff and internal
medical leadership as needed Requirements Board-Certified Nurse
Practitioner (AGNP, FNP, ANP) licensed in Pennsylvania Experience
with adult/geriatric populations in primary care, post-acute, or
SNF settings preferred Comfortable working independently inside
