Hours per Shift: Monday - Friday 8:00 AM - 4:30 PM; weekends as
needed Experience: Minimum of one year experience in Long Term
Care/Home Care setting License: Possession of a current state
license in Physical Therapy Certifications: BLS (AHA ONLY)
Must-Have: - Home health experience within the last year - One year
of PT experience in a home health setting Description: Provides
case management and direct patient care within a team environment.
Administers comprehensive physical therapy modalities in the home
setting. Assists the physician in evaluating the level of function.
Helps develop the plan of care and revises it as necessary.
Prepares clinical and progress notes. Advises and consults with the
family and other personnel. Participates in in-service programs.
Participates in assuring that Care Delivery Services are consistent
with the agency vision, mission, values, and strategic goals.
Promotes effective working relationships and works effectively as
part of the team. Utilizes resources, supplies, and time
efficiently to contain costs. Promotes positive, supportive, and
respectful communication to all external sources, patients,
families, and staff. Displays behaviors in support of the overall
organization. Supports public relations and marketing initiatives
to the local community. Strives for continuous improvement in all
aspects of the Physical Therapist function. Develops and maintains
a customer-oriented environment responsive to the needs of all
internal and external customers. Direct Patient Care Duties:
Develops and revises plans of treatment and discharge in
conjunction with the patient, family, physician, and care delivery
team. Provides an environment that promotes respect for patients,
their privacy, confidentiality, and property. Initiates appropriate
rehabilitative programs. Provides supervision of assistants to
ensure patient health, safety, and compliance with the plan of
care. Educates the patient, family, and team members regarding
disease processes and self-care techniques. Implements home therapy
programs, including gait training, transfers, activities of daily
living, home modification, care, and use of equipment. Periodically
evaluates patient progress and makes necessary adjustments to the
physical therapy program. Instructs para-professional staff in body
mechanics, use of equipment, bed mobility, transfers, gait
patterns, and home exercise programs. Coordinates, evaluates, and
supervises the quality of patient care services provided by the
home health agency. Interacts with patients, physicians, referral
sources, and others in a manner conducive to continued positive
relationships. Displays exemplary patience, courtesy, and tact.
Supports care management philosophy, focusing on customer-oriented
planning and case management. Participates in regularly scheduled
patient conferences with healthcare team members to discuss patient
care and ensure a coordinated plan of treatment. Assists in the
orientation of new rehabilitation personnel. Ensures the completion
of all appropriate clinical records for compliance with
regulations. Maintains current knowledge of available services and
resources. Demonstrates flexibility and willingness to cooperate
while working with customers and others. Addresses problem
resolution at the point of contact. Makes constructive
recommendations regarding ways to improve quality of patient care
and overall operations. Sets an example of integrity in all
interactions. Ensures patient needs are met through the care
delivery process. Ensures proper authorization and release of
information forms are signed by the patient or responsible party.
Participates in attaining operational and financial objectives.
Monitors results and progress and takes corrective action on
quality indicators. Understands the impact of the Physical
Therapist function on patients and care providers. Pursues efforts
to reduce or eliminate avoidable costs and errors. Develops quality
plans of care consistent with care paths and complies with
regulatory practices and guidelines. Responds to findings from
quality management processes and ensures resolution of potential
problem areas. Contributes to an environment of professional
growth, learning, trust, and mutual respect for all employees.
